Key Responsibilities
- Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
- Delegate and direct all discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities.
- Complete thorough medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines.
- Ensure all care is provided under signed, valid physician orders and within applicable insurance approvals.
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Contribute to organizational performance improvement initiatives and apply continuing education to meet strategic objectives.
Required Qualifications
- Active and unrestricted Physical Therapist license in the state of Indiana.
- Adherence to the Indiana Physical Therapy Practice Act and all applicable regulatory requirements.
- Ability to complete comprehensive patient evaluations and develop goal-directed treatment plans focused on progression toward discharge.
- Commitment to patient and employee safety standards, including safe patient handling and infection control practices.
- Strong clinical documentation skills in compliance with state, federal, and intermediary guidelines.
Preferred Experience / Skills
- Prior experience in an outpatient physical therapy setting.
- Experience supervising or mentoring physical therapy students.
- Active involvement in continuing education and professional development activities.
