Physical Therapist Location: Lake In The Hills, IL Agency: Jackson Therapy Partners Pay: $1,816 to $2,016 per week Shift Information: 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team in Lake in the Hills, IL, providing outpatient care for patients recovering from injuries and managing chronic conditions. This role evaluates assigned patients, develops and implements individualized treatment plans, documents care in accordance with regulatory and clinic requirements, and participates in student education and professional development. Maintain professional licensure, uphold organizational safety priorities, and contribute to quality improvement initiatives.

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in outpatient physical therapy preferred, including evaluation, plan of care development, goal writing, and functional treatment progression.
• Ability to complete timely, accurate medical record documentation, including letters of medical necessity and charge entry.
• Familiarity with state Practice Act, regulatory requirements, and professional standards of conduct.
• Willingness to participate in student supervision and clinical education as assigned.

Preferred Qualifications

• Previous travel or contract therapy experience in outpatient settings preferred.
• Experience with performance improvement initiatives and electronic medical record systems preferred.

Assignment Details

• Shift pattern: 5x8 (five 8-hour shifts per week).
• Workdays: Monday through Friday.
• Location: Outpatient clinic in Lake in the Hills, IL. Candidate may be expected to float to other nearby facilities as needed.
