American Traveler is seeking a travel nurse RN Case Manager, Skilled Nursing for a travel nursing job in Charlotte, North Carolina. Job Description & Requirements Specialty: Skilled Nursing Discipline: RN Start Date: 07/06/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Assignment Overview Shift: Days, 5x8hrs Hours: 40 hrs/wk Start Date: Jul 6, 2026 Length: 13 weeks Openings: 1 Description American Traveler is seeking an experienced BSN-prepared RN for an MDS Coordinator position within a hospital-based Skilled Nursing Facility, requiring at least 2 years of hands-on MDS assessment experience and an active NC or compact RN license. Details Hospital-based Skilled Nursing Facility (SNF) unit accepting residents for short-term rehab stays of 14–21 days Patient population includes a wide variety of diagnoses ranging from generalized weakness to surgical aftercare, with a large portion being post-surgical care and wound/pressure ulcer management Patient ratio of 1:12 with an average daily census of 9 and 12 beds on the unit Schedule is 5x8-hour day shifts, 8:30 AM – 5:00 PM No call, float, or weekend requirements EMR systems in use: Epic, Epic Chat/Voalte, and Rover; Pyxis medication dispensing system also utilized Unit does not utilize restraints Requirements Active NC or compact RN license required BSN required Minimum 2 years of relevant RN experience required Current, hands-on MDS assessment experience within the last 3 years required — must be direct assessment completion, not a management role Current BLS certification required Required work setting experience: Long-Term Acute Care/SNF, Managed Care, MDS Coordinator, and Rehab Required process skills: Admission Criteria, Appeals and Denials, Audits, Benefits Eligibility, Care Coordination, Clinical Documentation Improvement, Medical Necessity Determination, Discharge Planning, DME/Home Health/Hospice/SNF Needs Assessment, Physician Advisor, Plan of Care, Pre-Cert Review, Prior Authorizations, QI/CASPER Review, and Retrospective Review Required regulatory knowledge: CMS, CPT coding and billing, Department of Health, ICD-10 Coding, and MDS Age-specific/population-based care experience required Triple check and utilization review experience required Two professional references from a manager or supervisor within the past 2 years required (pending at time of consideration is acceptable) Additional Information Responsibilities include MDS completion, baseline care plan development and presentation, care plan meetings, CAA completion, assessment submission and validation, oversight of certification/recertification process, ICD-10 coding for new admissions, MDS change education, triple check/utilization review, and ensuring UB-04 accuracy Additional duties include admission referral review and insurance preauthorization for new admissions as needed to support Social Work Orientation consists of one week of hospital orientation followed by 24 hours of unit-specific orientation; no RTO is permitted during the first week of the assignment Pre-start online modules are required (approximately 4–5 hours for non-acute roles) and must also be completed annually Candidates must work 2 out of 3 major holidays RTO of 7 days or fewer may be auto-approved as long as it does not fall on a holiday; more than 7 days requires facility approval; no RTO is permitted during the first week of orientation Any previous permanent, PRN, or EP staff employee of Novant Health or New Hanover Regional must wait 12 months before applying as a traveler; facility rehire approval is required Local candidates within 50 miles of the facility are accepted Navy blue scrubs required; scrubs are not provided Travel pairs are allowed — each must have their own transportation and be comfortable with potentially different schedules First-time travelers are welcome to apply RAC-CT and ICD-10 certifications preferred Preferred regulatory knowledge includes DRG, HIPAA, NCQA, OBRA, and RAPS LTAC experience preferred American Traveler Job ID P-715699.
