Job DescriptionJob DescriptionPatient LiaisonKeta Medical CenterPay rate: $31.25 -$33.65 per hourStatus: Full Time, Benefits EligibleSchedule: This position requires flexibility that will typically require working one weekend day per week. For example: Sunday-Thursday, or Tuesday-Saturday. The hours may vary from 9am-5pm or may require later hours from 11am - 7pm.
Location: Keta Medical has locations throughout the New York metro area. Ability to reach all locations is required. Depending on your home location, travel expenses will be reimbursed for sites that require extensive driving and/or public transportation costs. Keta Medical Center is hiring a full-time Patient Experience Liaison to join our growing team. This on-site position plays a vital role in ensuring patients feel supported, informed, and cared for throughout their treatment journey. The role includes regular travel to our clinic locations and combines patient communication with administrative coordination.
Responsibilities include • Serving as a primary point of contact for new and existing patients, answering questions and providing clear, empathetic information about treatment and scheduling • Greeting patients at their first visit and offering in-person support across clinic sites • Tracking patient progress during the induction phase and following up on any missed or delayed sessions • Assisting with care coordination, therapist assignments, and communication across programs
Responsibilities:
- Serving as a primary point of contact for new and existing patients, answering questions and providing clear, empathetic information about treatment and scheduling
- Greeting patients at their first visit, conducting comprehensive orientations that review program structure, introduce clinic staff and resources, and outline what to expect during each phase of treatment
- Educating patients on available programs, psychiatric services, therapy modalities, and treatment options to help them make informed decisions about their care and understand the full scope of support available to them
- Tracking patient progress throughout the entire duration of their care, proactively following up on any missed or delayed sessions and identifying potential barriers to engagement
- Assisting with care coordination, therapist assignments, and scheduling logistics across programs
- Collaborating with clinical teams including physicians, nurses, and therapists to support patients who may need additional attention, contributing observations and helping coordinate individualized care plans as needed
- Communicating patient needs and concerns to clinical staff in a timely manner, ensuring relevant updates are shared across the care team to support continuity of care
- Attending regular team meetings to review upcoming patient sessions, debrief on completed treatments, and align on individualized care considerations including patient readiness and any clinical factors relevant to their continued progress
- Participating in community-based events as needed to represent the practice, build awareness of available services, and support outreach efforts
- Bachelor’s Degree in Social Work, Public Health, Psychology, Healthcare Administration, or a related field
- Familiarity with EHR/CRM systems (Salesforce)
- Strong data-tracking skills (comfortable using spreadsheets or dashboards to monitor patient metrics)
- Community outreach, preferably in mental health setting
- This position requires closely working with mental health patients. A positive attitude, warm energy, and flexibility are needed.
